I hereby give permission for the children listed above to attend Champlain Valley Baptist Church and to participate in church activities. I acknowledge that in attending Champlain Valley Baptist Church, the children listed will be instructed according to the church’s teachings. In signing this permission slip, I release Champlain Valley Baptist Church, its officers, and any volunteers of liability from loss, injury, or any other harm arising in connection with the above listed children’s participation in Champlain Valley Baptist Church. I authorize the church and its volunteers to make all necessary medical decisions for the above listed children in the event that I cannot be reached.
